    We are now hiring for a Transition Coordinator.  This is a newly created position that will collaborate with the Education Leadership Team, Vocational Services Department, and classroom teachers to determine the individualized transition needs of eligible students.




    • Develop, implement, and monitor a school-wide plan to provide appropriate and effective transition services to meet the individualized transition needs of each eligible student.
    • Collaborate with IEP teams to write meaningful transition plans, determine post-secondary goals and transition services, and/or serve as a resource to special education teachers. Attend IEP meetings to discuss transition plans/options.
    • Facilitate tours of workplace settings and/or post-secondary institutions to provide exposure and promote enrollment/employment.
    • Coordinate bringing the workplace/living environments into the classroom to provide a meaningful context for learning in all subject areas (curriculum, structured work systems, presentations by employers, etc.)
    • Conduct informal and formal transition assessments and/or monitor the administration of transition assessments administered by education staff to identify strengths, needs, preferences, and interests.
    • Provide consultation to teachers/support staff around post-secondary education, independent living, and vocational opportunities for all students eligible for transition services.
    • Collaborate with Vocation Department in the areas of exploring and identifying community volunteer and paid vocational opportunities.
    • May be involved in interviewing and selection, orientation and training. Supports staff in their professional development.


    • Bachelor’s Degree in Special Education or related field required.
    • DPI Special Education licensure required.
    • Transition Coordinator Certification required within 2 years.
    • Three years experience in an education setting required.
    • Demonstrated experience in working with students enrolled in special education programs.
    • Must have a valid WI driver license in good standing and qualify to be an agency approved driver.
